Referrals to Therapy Collective
At Therapy Collective, we accept a range of referral pathways to support individuals and families in accessing psychology services. Below you’ll find information on the different options available, what’s required, and how to proceed with referring yourself or someone in your care.
Referral Pathways

Completed by a GP
Entitles clients to Medicare rebates (up to 10 sessions per calendar year)
MHCP must be provided before the appointment to claim rebates
GP can fax, email, or send the MHCP directly

We accept self-managed and plan-managed participants
Please send relevant NDIS documents (plan, reports, goals)
Funding may apply for individual therapy and some assessments
Support Coordinators may send referrals directly

Private fees apply
Some private health insurers may offer rebates
Suitable for clients who prefer direct access

Requires GP referral and case manager approval
Case manager details must be included
Insurer may request reports or treatment plans

Appropriate DVA referral form required

How To Send A Referral

Prepare Your
Documents
Gather any relevant paperwork such as:
- Mental Health Care Plan
- NDIS plan or supporting documents
- GP referral letter
- Case manager approval (if applicable)
- Client details (name, DOB, contact info)
Everything can be uploaded or emailed directly to our team.
03.
Send Your
Referral
Once you have your documents ready, send them through to our admin team.
You can also send referrals via fax (if applicable) or upload them through our contact form.
