Choose Your
Referral Type

Review the referral options above to determine which pathway applies (e.g., Mental Health Care Plan, NDIS, Private, Workers Compensation, DVA).

Prepare Your
Documents

Gather any relevant paperwork such as:

- Mental Health Care Plan
- NDIS plan or supporting documents
- GP referral letter
- Case manager approval (if applicable)
- Client details (name, DOB, contact info)

Everything can be uploaded or emailed directly to our team.

Send Your
Referral

Once you have your documents ready, send them through to our admin team.

You can also send referrals via fax (if applicable) or upload them through our contact form.
